First Visit
At your first appointment, you will meet with Dr. Rudolph Bolling for a thorough Diagnostic Evaluation. Dr. Rudolph Bolling may also use various Psychometric Tests to further clarify her assessment and reach a reliable diagnosis.
We ask that you fill out the Patient Information Form (PIF) before your visit. This questionnaire consists of detailed information, which you may not feel comfortable discussing in front of your child or family member. The Patient Information Form MUST be completed before an appointment will be made.
A copy of all required forms can be found on our Forms page. These include the Patient Information Form, Permission to Treat, Initial Parent and Teacher Vanderbilts, Release of Information, Privacy Notice (HIPAA), and Financial Agreement. These should all be completed before the appointment with Dr. Rudolph Bolling.
Appointment Request
We schedule appointments directly with the patient or the patient's caregiver (if under age 18). All additional information from current mental health and primary care physicians (PCP) are welcome.
If the patient has Medicaid, the PCP will also have to complete a referral stating the last date that the EPSDT was completed. No appointments can be made until this form is received.
Please be sure to complete the Patient Information Form and return it to our office. We WILL NOT schedule an appointment until we receive this form AND the Referral/ EPSDT from your primary physician. (NOTE: The referral is only needed if your child has Medicaid.)
